provide short-term intensive community based care as a viable alternative to hospital admission
engage service users and their families/carers in tailored programmes of care and promote the stabilisation of an individual's mental health within their home environment
In providing alternatives to hospital based care and in the work of the team to engage and empower service users and their families this project provides an excellent example of how principles can inform service design, development and delivery.
