Mental Welfare Commission forms
The Mental Health Act regulations require hospital managers to make certain reports to the Mental Welfare Commission. We have also identified monitoring priorities which require services to provide certain information to us. Forms have been developed for this purpose. Young people
Notification of the admission of a child or a young person (up to their 18th birthday) to a non-specialist unit for the treatment of a mental disorder ADM 2 (.pdf).
Retrospective quarterly reports are to be completed at the end of the month following the quarter e.g. report due 31st January for the period 1 October to 31 December. A pro-forma has been provided for these notifications ADM 3 (.doc).
Specified persons
'Specified persons' may be subject to restrictions of correspondence or telephones, and other measures taken to ensure safety and security in hospitals.
Notification to the Mental Welfare Commission of the designation of a specified person RES 1(.pdf). This decision will last for 6 months. All patients in the State hospital are automatically 'specified persons' in relation to 'safety and security in hospital' measures, telephones and mail, therefore this notification to the Commission is not required. All patients at the Orchard Clinic are automatically considered 'specified persons' in relation to 'safety and security' measures, therefore notification of decisions is only required in relation to use of telephones and mail. This also applies to any other medium secure unit which may be announced in regulations. At the end of six months, another RES1 form may be sent to the Commission.
Notification to the Mental Welfare Commission of a reasoned opinion review of a specified person RES 2 (.pdf). This form should be sent to the Commission every time an RMO is asked to review the status of a 'specified person' by the patient or named person.
Restriction of communications
Notification to the Mental Welfare Commission of measures to restrict the use of telephones RES 3 (.pdf). This form is to be used to notify the Commission when measures are taken to restrict the use of telephones by patients during a period the person is 'specified'. Mobile phones are not included.
Review of prohibitions and restrictions on the use of telephones RES3A (.pdf). This form is to be used where a patient's RMO has restricted or prohibited the patient's use of a telephone, and the specified person (the patient) has asked for it to be reviewed.
Restriction of correspondence
Notification to the Mental Welfare Commission of measures to withhold mail from/to patients RES 6 (.pdf). This doesn't include email correspondence.
Safety and security in hospitals
Safety and security annual incidence and circumstance report to the Mental Welfare Commission RES 4 (.pdf) and continuation sheet RES 5 (.pdf). These forms are used to collate figures of annual incidence of measures taken on wards. Reports are to be completed at the end of April each year for the period between 1 April and 31 March the previous year.
